What is a deficiency?

Actual Harm Deficiency A violation that results in harm for a resident. These range from isolated cases to widespread harm.
Deficiency Comments

These written comments describe the nature of actual harm violations, although they may also detail other deficiencies, such as call light violations.

Substandard Quality of Care A serious violation of harm regularly resulting in a fine. The administrator has the deficiency marked on his license. The home could also lose the ability to train its own staff for two years and would then be reliant on the state.
Immediate Jeopardy The most severe deficiency. It places the home on a 23-day termination track. If the home does not fix the deficiency by then, the government will revoke its Medicaid license. This deficiency also involves fines.
