| Substandard Quality of Care |
A serious violation of harm regularly resulting in
a fine. The administrator has the deficiency marked on his license.
The home could also lose the ability to train its own staff for two
years and would then be reliant on the state. |
| Immediate Jeopardy |
The most severe deficiency. It places the home on
a 23-day termination track. If the home does not fix the deficiency
by then, the government will revoke its Medicaid license. This deficiency
also involves fines. |
